Free and Discounted Athletic Physicals
Middle and high school athletes may receive discounted athletic physicals through Milford Family Care on July 7 or August 4, 2010. This service is free to current patients and is discounted to $35 for new patients. Current patients can also request a hemoglobin or urinalysis test for an additional $10 each and any immunizations will be given at an additional charge. Payment is due when services are rendered. No insurance claim will be filed for these special rates.

This service is free to current patients and is discounted to $35 for new patients.
