Responsibilities:

• Deliver comprehensive skilled nursing care to patients within the home setting
• Participates in the development and implementation of patient care plans in collaboration with physicians and the interdisciplinary team
• Provides quality end-of-life cares and support to patient, family, and caregivers 
   

Requirements:

• Flexible, multitasker, demonstrates effective communication and assessment skills
• IA RN license
• BLS required
• Valid driver's license and proof of insurance required

Schedule: 

• 72 hours a pay period - 8 hour shifts - Days (Full-time benefit eligible)
• Typically one holiday of on-call only per year
• Typically one weekend of on-call only every 6 weeks
• The schedule may fluctuate based upon the needs of the department
