REGISTRATION:
Register by sending registration form with payment to Spencer Hospital Human Resource Development department. Registration requested by 8am Aug 18, 2023. No late registrations or drop-in’s permissible. Course maximum is 30 participants.
CANCELLATION:
Cancellation notification received prior to 8am Aug 18, 2023 will receive a full refund. Cancellations received after Aug 18, 2023 will receive no refund. Spencer Hospital reserves the right to cancel this program if there is insufficient enrollment. In this event, every attempt will be made to notify enrollees prior to the event.
FEES:
Spencer Hospital Employees who have Director Approval: No fee
Non employees of Spencer Hospital: $450
Fees includes course materials, meal and snacks.
The AAFP has reviewed ALSO Provider Course and deemed it acceptable for up to 8.00 In-Person, Live (could include online) AAFP Prescribed credit. Term of Approval is from 09/29/2023 to 09/29/2023. Physicians should claim only the credit commensurate with the extent of their participation in the activity.
The American Academy of Family Physicians is approved as a provider of continuing nursing education by the Kansas State Board of Nursing. This course offering is approved for 9.60 contact hours applicable for APRN, RN, LPN, or LMHT relicensure. Kansas State Board of Nursing provider number: LT0278-0312. This is an advanced level course and may not be suitable for all general continuing nursing education.
