Epic MyChart
Spencer Hospital offers a secure, convenient way to access your health information online through Epic MyChart. MyChart allows you to view healthcare information from Spencer Hospital and clinics from your computer or mobile device, as well as other healthcare organizations that use Epic MyChart.
With MyChart, you can:
- View test results and health information
- Request and manage appointments
- Review discharge instructions and provider notes
- Update personal and medical information
- Share access to your medical record with family members securely
Your former AveraChart portal will provide view-only access for a limited time. Most information has been transitioned to your new MyChart portal. Epic MyChart Registration FAQ
Patients ages 18 and older can have MyChart accounts to manage their care and access their medical record.

Teenagers aged 14-17 can enroll in their own Avera MyChart account to manage their care. With MyChart, teens can schedule appointments and take part in a video visit, join waitlists, send messages to their care team, manage prescriptions, and view test results and other health information.
- Teens must create an account on their own. Parents should not create one for them.
- By law, parents and legal guardians will automatically have access to the children’s medical record, with some differences by state.
- Access is not allowed for parents of emancipated minors.
- There are various other relationship categories to consider in relation to medical record access.

Parents can gain proxy access to your child’s patient record using one of the methods below.

- If you’re the legal guardian, bring legal documentation to your clinic.
Parents and legal guardians will have access to a child’s medical record as a proxy unless the child is an emancipated minor, or access has expired. You must have your own MyChart to gain proxy access. You can register below on this page.

If you help manage someone else’s care, like a child, elderly parent or a dependent, proxy access lets you view their health record and coordinate care. This can make things easier for you and your loved one.

How to Gain Proxy Access For A Child:
- Call the Avera help desk for MyChart at 855-667-9704.
- Talk to staff at your clinic. Call us or ask about it at your next appointment.
- If you’re the legal guardian, bring legal documentation to your clinic.
To Manage Proxy Access for an Adult:
- Go to Manage friends and family access in MyChart. (You’ll need to log in first.)
- If you’re already logged in and want to navigate to the Sharing Hub, select the top left menu icon. Look for and select the Sharing Hub menu option. Then choose Manage friends and family access.
- The Friends and Family Access page will show the following, if they’re set up:
- The names of anyone else who can see your health info
- The names of any person that you have access to their health info
- To add someone and give them access to your health info:
- Select Invite friends or family.
- Complete the on-screen steps for the invitee name and email address. Choose their access level.
- View the Terms and Conditions, and select I agree. Select Send invite.
- The invitee will receive an email from Avera MyChart to log in and accept the invitation, and they must verify they know you by entering your date of birth.
- Once they’ve confirmed, you’ll be notified that they accepted your invitation.
- To remove someone as a proxy for your medical record:
- Go to the section showing who can see your record.
- Next to that person’s name and choose Revoke. Click Revoke again to confirm.
The easiest and fastest way to sign up for MyChart is by using an activation code provided by Avera. Many patients receive their code in a communication from Avera with the MyChart launch, when they have an upcoming appointment or another care touchpoint with us.
