Registration

For anticipated hospital stays, pre-registration is encouraged so that when you arrive at the hospital your registration process will be a quick validation and check-in.  If your admission is unanticipated, you will be registered during an acceptable time in your treatment plan.

To pre-register, please meet with a business office representative located on the first floor of Spencer Hospital any weekday between the hours of 8 AM – 4:30 PM. Please have the following items and information with you:  

• Driver’s license or official photo identification card
• Insurance cards (All insurance – primary , secondary)
• Additional payer information (work compensation, accident)
• Personal information (name, date-of-birth, phone)
• Email address
• Employment information
• Emergency contact
• Family physician
