Empowering Our Community Through Diabetes Education

Diabetes affects many individuals in our community, and receiving a diagnosis can feel overwhelming. At Spencer Hospital, we’re here to make sure no one has to navigate it alone. With the right knowledge, resources, and support, people living with diabetes can take control of their health and lead full, active lives. 

Understanding Diabetes 

Diabetes occurs when the body can’t properly use insulin, causing glucose to build up in the bloodstream. Over time, high blood sugar can contribute to serious health concerns, but with proper management, diabetes can be controlled successfully. Types include Type 1, Type 2, gestational diabetes, and pre-diabetes. 

Our Education & Support Program 

We offer a comprehensive Diabetes Education and Self-Management Program designed to help you understand your diagnosis and build confidence in managing it. After an initial assessment, patients participate in personalized education by meeting with our certified diabetes educators either one-on-one or in small groups to cover meal planning, blood sugar monitoring, medications, physical activity, and complication prevention. 

A Team Centered on Your Health 

Our diabetes care team includes a nurse educator, a registered dietitian, and your family physician, working closely together to create a plan tailored to your needs and lifestyle. 

Get Started with Support 

Most patients are referred by their family physician, and diabetes education is covered by Medicare, Medicaid, and most insurance plans. Once we receive your referral, we’ll reach out to schedule your initial assessment and begin building a plan that fits your goals.
